We are formally seeking professional input to this case study. This web page contains detailed information on Yafaau's medical history. My Son, Ahmed Yafaau Yasiph was born on January 4, 2003 in Maldives(IGM Hospital). Due to prolonged second stage of labour, he was born with no breathing, no heartbeat and doctors confirmed that he suffered severe Asphyxia(HIE) at birth followed by brain injury which effects his developmental milestones and now has seizures like convulsions and is now diagnosed as Cerebral Palsy.
 
 False Report
 
Although Yafaau's report said he was delivered by Vacuum extraction (Doctors noted as "a difficult extraction" Ref: Resuscitation note) due to prolonged second stage, it was a natural birth. At birth he was cyanosed, had no cardiac or respiratory activity. Oro-Nasal suctioning was done and bag/mask ventilation was started within first minute after birth and cardiac massage was given.
 
Heart rate picked up and remained throughout the remaining period of resuscitation. Within 24hrs after birth seizures and apnea was noted. At sixth day baby went to have a CT Scan which reported
  • 1 .Chephal haematoma in right temporal and parieto-occipital regions.
  • 2. Hypoxic changes in cerebral parenchyma Ref: CT Scan 09-04-2003

He had involuntary movements and had stiffness in his limbs from birth. His first hours and days were spent in the Nursery(NICU). He was not taking any feeds, there was no sucking present. So from Birth Yafaau required Tube Feeding. Till 2 months he was on Expressed breast milk via tube.
 
Almost after one month at discharge doctor noted his condition also as hypertonia, poor feeding and involuntary movements Ref: Discharge Summary 02-02-2003
 
We went to Sri Lanka on 1st of April and take further evaluation which did not have any findings of Hypsarrhythmia in those EEGs Ref: EEG1,EEG2 But noted his involuntary movements as Infantile Spasm(Salaam Attacks, I'll refer to these as convulsions within the site) Ref: Discharge Summary 10th April. Started Clonazepam 1/4th tabled twice daily. We started to note the number of convulsions. Convulsions charts.
 
Number of convulsions started to increase from June till August. Went to Apollo, Sri lanka again and found that Yafaau had hypsarrhythmia on EEG's Ref: EEG3,EEG4 Yafaau was given ACTH shots which followed high spikes of fever and had vomit feeling. Stopped ACTH after 14 days. Convulsions reduced noticeably. Yafaau was still was vomiting at the time of discharge. We also showed to Nawaloka Hospital and followed another Pediatricians advice too. On discharge we did note another form of convulsion-like movement which both doctors(Dr P.M, Apollo Hospital and Dr S.D. Silva, Nawaloka Hospital) told that it was not a convulsion. We were advised to give him therapeutic exercises and give stimulation with the anticonvulsants. Clonazepam(Rivotril) 1tb morn. 1tb night and Sodium Valproate(Valparin 200mg) 2.5ml morn 2.5ml night.
 
20031127: At the moment we are still continuing these medicines with home based exercises. We started going to the Physiotherapy Dept. on 27th November 2003. We continue Physiotherapy with Life skills therapy at CARE society. We have the special seating with the Aid from Theemuge. We just came from India this Sep 25th after making splints for Yafaau. We did a new EEG which I will upload soon.(India Trip details)

20050710: Convulsions seem to be less in the cluster but its intensity is higher than his first year. Still on anti-convulsions and new videos available.
 
20070512: Yafaau’s doesn’t have those horrible convulsions anymore, Alhamdhu Lillahi. It stopped on 2005 November but we still give him the anti-convulsions. But we managed to stop S.Valproate after one year.

Medicines now on:

1)    Lioresal(Baclofen) 10mg 1/2 tablet a day
2)    Rivotril(Clonazepam) 0.5mg 1 tablet a day
